Study Abroad Applications -- the Home School Nomination/Home School 
Approval Forms
For those of you who are in the process of completing study abroad 
program applications, a few notes on the approval forms included with 
most applications ...

1.  These forms have various names, such as Undergraduate Approval 
Form, Home School Nomination, Home School Approval Form, etc.  All 
are essentially the same thing -- a form that asks questions about 
good academic standing, if you've been approved by Yale to study 
abroad, and how the credit transfer works.

2.  For academic semester/year study abroad:  these forms need to be 
filled out by one of the study abroad advisors in IEFP.  We request 
that you bring these forms in to IEFP, along with an addressed, 
stamped envelope, and leave them for us to fill out (you can also 
give them to us during appointments or walk-ins, but we will still 
need the envelope to send them out).  Please be sure you fill out 
your portion of the form before leaving it for us.  We can usually 
get these forms completed and in the mail within  two business days.

3.  For summer study abroad:  in the case of summer applications, it 
is usually most appropriate for these forms to be filled out by your 
residential college dean (she/he may call our office with any 
questions about the form).  This is because there is no approval 
process through the JYA Committee for summer study abroad, and our 
Committee does not play an official role in awarding summer credit.

4.  Specific info for CIEE and Butler forms:  CIEE forms have two 
parts -- one is a general section asking if the student has their 
home school's approval to study abroad, the other is a more in-depth 
evaluation of your adjustment to college life, etc.  IEFP will fill 
out the first part of these forms, but will ask that you have your 
dean or academic advisor fill out the second part.  For Butler forms, 
please make sure you leave both pages of the form with this office -- 
the first page has your information, including the programs to which 
you are applying, and the second page is the part we will out.  We 
(and Butler!) need both these pages to stay together.
