Miyagawa Kazuo

That English title probably refers to the book

"Kyameraman Ichidai: Watakushi no Eiga Jinsei 60-nen,"

Miyagawa Kazuo's memoirs, written by the great cinematographer in 1983-84
and published in 1985 by PHP Kenkyuujo.
I don't think the book has been translated or published in English.

There was also a film-biography of Miyagawa, produced and broadcast by NHK
around 1990 or 1991.  This television program may have had the same title,
but I'm not sure of that.

I get this reference  "Miyagawa Kazuo: My Life as a Cameraman"
but I don't know what it is.
