Hi,
 
1)  According to numerous Google entries, his name was Ikeuchi Yoshitoyo.

2)  Considering Ito Daisuke's mother's horror at the idea of her son, a descendant of samurai, entering the film business (and making him swear never to become an actor), it is no surprise that Itami took a "stage" name.  His son took the name, quite reasonably, as the "business" name of his family.
