Dear FAST Subcommittee,

Thanks to everyone who sent input already about converting to an interest group or not and the name and charge for that group. Now I really need everyone to consider the questions below and send their responses so that we can move forward in one direction or another. 

1. Please choose one: 
	_____ I prefer that the group remain a subcommittee of SAC devoted exclusively to FAST. 
	_____ I prefer that the group become an interest group of CCS and remain devoted primarily to FAST. 
	_____ I prefer that the group become an interest group of CCS with a focus extending to applications and aspects of faceted subject access including but not limited to FAST. 
	_____ Other (please specify)

2. If the consensus of the group is to become an interest group, I support the following name and charge: 
	_____ "Faceted Subject Access Interest Group, with charge: To explore the theory and applications related to subject terminology intended for faceted application, including FAST (Faceted Application of Subject Terminology), AAT (Art and Architecture Thesaurus), LCGFT (Library of Congress Genre/Form Terms), and others. To provide a forum for users and others interested in the faceted approach to subject access to compare notes and discuss further developments and implementations of subject faceting for digital projects and related topics."
