CoopCat has posted the document at 
http://www.loc.gov/catdir/pcc/scs/scsresponse_dates.html (thanks, Ana!); it 
is also accessible off the PCC home page and the SCS page. I understand 
that CPSO has received on the order of 150 responses to their request ( 
http://www.loc.gov/catdir/cpso/pndates.html). I'm glad to know that many 
folks took the time to respond!
