A while ago there was a message on the listserve
for DC Comics Vertigo (publisher of _Sandman_)
containing a link to a very detailed interview 
with Neil Gaiman on how he went about creating
the dialogue and doing the subbing for Mononokehime.
Naturally now I can't find the URL, but I'll
keep looking. In the meantime, here's a long,
somewhat more superficial interview that deals
with some of your questions:

The first part of the interview is about a new
_Sandman_ project that is also Japan related,
_The Dream Hunters_, but the last half of the 
article deals with Mononokehime. Gaiman used
the subtitled version for the basic translation
and clearly indicates the he sent all the dialogue
to Miyazaki for his approval.
