There are numerous companies that supply tag stock of this kind - you would be able to specific the size and preprinted parts of the tag. The company we have used before is Allen-Bailey Tag and Label Inc. of One Maine Street, Whitinsville, MA 01588. Their web address is http://www.abtl.com/
Hope that helps

Subject: [NHCOLL-L:3935] specimen tags for museum skin specimens (mammal and bird)
Hello,
We are trying to order archival specimen tags for museum study skins.
The company that we had last ordered from has sold that part of their
business and the new owners have a "gray" colored material that they
use. I want something white or off-white to hand write the information
on. We also would like our museum's name printed on them. Does anyone
have a source for these type of tags? I have searched the web and
cannot find anything. Thanks for any information you can supply.
