[yul-naco] PCC statistics form

Some of you are continuing to use the old PCC 
statistics form (the one that says RLIN21). Just 
a reminder that we put up a new form on the website (this one says OCLC):
http://www.library.yale.edu/cataloging/authorities/PCCMonthlyStatisticsForm.xls

No need to resubmit statistics that you have 
already turned in, but if you keep a "master" 
copy, it would be good to download the updated form and use that from now on.

If you try to access the form using Firefox, you 
might need to clear the cache first (I had 
troubles with this and kept getting the old form 
even though I knew the new one had been posted!).

To clear the cache:

Go to Tools>Options.
Click the "Privacy" icon.
Click the "Cache" tab.
Click the "Clear Cache Now" button.
Click "OK".
